Job Description
GI Oncology Nurse Navigator

Under general supervision, serves as a leader, change agent and coordinator of patient care services for GI Oncology patients. Takes a leadership role in overall design and implementation of a patient centered care delivery system that facilitates achievement of patient outcomes, supports continuity of care and enhances patient and care giver satisfaction. Accountable for the processes and outcomes of care during an episode of care. Accountable to peers for the advancement of group practice and to the supervisor for the totality of work performance.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  1. Serves as a role model and clinical resource for expert care and outcomes management.
  2. Assists staff in developing and improving skills in managing therapeutic relationships with patients and families.
  3. Assesses staff performance in documentation and provides feedback. Monitors improvement plans. Develops and implements systems to improve documentation.
  4. Assesses and improves preoperative patient instructions. Responsible for patient and family education. Monitors processes and outcomes of patient education; implements improvements as appropriate.
  5. Develops strategies to promote continuity, consistency and confidentiality of patient information across organizational boundaries. Develops systems to streamline and improve patient information management processes. Coordinates multidisciplinary approach to screening patients and arranges multimodality therapy.
  6. Coordinates weekly multidisciplinary GI Oncology clinic and conference. Participates as an active and assertive member of the multidisciplinary team. Assesses and develops staff expertise in functioning in a collaborative role within this team. Takes the lead in ensuring continuity and consistency of care. Incorporates feedback from team members into improvements in unit operations which support patient care.
  7. Develops and manages unit level systems to coordinate multidisciplinary care to meet patient care outcomes.
  8. Coordinates patient admission process and arrangements for patient beds. Facilitates discharge preparation and procedures.
  9. Coordinates ordering and follow-up of lab work, diagnostic testing and surgical posting.
  10. Coordinates postoperative care with surgical and oncology nurse practitioners and other providers.
  11. Assesses resources and assists staff to appropriately set priorities for patient care. Identifies areas where work redesign would improve practice effectiveness and makes changes as appropriate.
  12. Works with supervisors to identify, anticipate and resolve issues related to patient flow in the clinic. Monitors and reports trends.
  13. Assists in non-operative, non-invasive clinical procedures including patient evaluation.
  14. Assists staff in exercising judgment in the follow-up of patient care issues, including use of physician and/or administrative chain of command. Analyzes trends and makes recommendation for improvement if necessary.
  15. Assesses eligibility for clinical trials with the physicians through review of medical records.
  16. Assists in preparation of grants and manuscripts.
  17. Assists staff to develop effective group dynamic and conflict resolution skills. Fosters open communication among staff. Demonstrates effectiveness as a group leader and facilitates the learning of other staff in leadership roles.
  18. Develops and monitors systems to review and discuss quality results, patient feedback, problem solving and development of plans to improve quality of care and service.
  19. Collaborates with supervisor regarding patient scheduling, staff development and credentialing, practice activities and quality of care.
